Managing your mental health care plan can be challenging. Our community support services provide a comprehensive support program that accompanies you throughout your treatment journey, particularly during transitions between treatment and independent living at home.
The Department of Behavioral Health outlines that Community Support programs will include:
- Participation in creating and executing a Consumer’s IRP/IPC and Community Support Individualized Service Specific Plan (ISSP)
- Providing assistance and support during stressful situations
- Offering mental health education, support, and consultation to individuals and their families
- Delivering individual mental health services and interventions to develop interpersonal and community coping skills adapting to home, school, and work environments
- Assisting with symptom self-monitoring and management to identify and reduce the negative impact of psychiatric symptoms on daily living, financial management, personal development, or school/work performance
- Enhancing social support skills and networks to reduce life stresses caused by mental illness and facilitate independent living
- Creating strategies and supportive interventions to prevent out-of-home placements for adults, children, and youth and to strengthen family support skills and knowledge of the individual’s strengths and limitations
- Developing mental health relapse prevention strategies and plans
